Patient Forms
Upon entering our office, our receptionist will welcome you and explain all that needs to be done on your first visit. We request that you complete our patient forms in the New Patient Center. This paperwork provides us with your health history and information on your condition.
Consultation
Next, you will have a consultation with Dr. Mark Swain to discuss your spinal health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Mark Swain to learn more about you, your condition, and your expectations to determine how chiropractic care can meet your goals.
Examination
After your consultation, Dr. Mark Swain will perform a complete spinal examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will also be performed. However, nothing will be done in our office without your consent.
X-Ray Studies
Depending on your specific condition, we may request X-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. X-rays are done at the Algonquin X-ray clinic and are covered by OHIP.
Report of Findings
Once the information is collected and examinations are performed, Dr. Mark Swain will give you a detailed report of all findings and answer any questions including:
- How can you help me?
- How often do I need to come in?
- What will my treatment cost?
After reviewing your health history, goals, and examination findings and X-rays, Dr. Mark Swain will discuss recommendations and notify you if your condition requires care with other health care providers. Dr. Mark Swain will provide the best treatment and restorative care program for your needs.
Treatment
After your report of findings, you can choose to experience your first treatment. This may include spinal adjustments, Cox Flexion decompression, and/or soft tissue therapy. Treatment is interactive, and everything will be explained beforehand, so you can express concerns about the different styles of treatment anytime.
Spinal self-care and Back4life workshop
Before leaving, the doctor will suggest self-care to incorporate outside of treatment, including ice or heat application instructions, certain activities or positions to avoid, and at-home exercises and/or stretches. Included in your program of care is our Back4life workshop. This 45-minute workshop will teach you the value of spinal health and help you better understand your condition and how to manage it. You will learn essential everyday tips to build a strong, healthy back for life!
